New patients are welcome!
Download and print our patient forms to save yourself some time on your first appointment. These forms can be emailed to us (preferred) or delivered at check-in.  We are also happy to print out forms for you upon arrival if needed.

Appointment Policy

To respect your time and ours, we operate on an appointment basis. Our schedule has emergency time built into each day. Occasional delays may occur due to unexpected emergencies; however, we make a sincere effort to stay on time. We understand that your busy schedule may change; therefore, we confirm your appointment a week in advance. If you do not call back to confirm your appointment or at your request; we will also provide a courtesy call the day before your appointment. Our office cancellation policy is 48 hours (two business days). If you are unable to honor your scheduled appointment, please notify us as soon as possible so that we may offer this time to another patient.

Our office strives to schedule appointments at your convenience and when time is available.

Accepted Insurance

We accept most dental insurance plans, as long as you are not required to see a specific dentist to have coverage.

We are “in-network” with Delta Dental. However, if we are out-of-network with your insurance provider, that does not mean you cannot receive treatment at our office. We accept out-of-network patients all the time and we’ll do everything we can to help you maximize your benefits. If you’re unsure about whether we accept your insurance, please don’t hesitate to contact our office. Our team can assist you in verifying your insurance plan.

Our office accepts checks, cash, debit cards, MasterCard, VISA, American Express and Discover CardCare Credit is a long-term financing option that offers no-interest payment plans.
